how do you kill with pvp off????
Who combat tagged you? As far as I am aware it is still possible to combat tag yourself. Personally I've done this through bowing myself, hitting myself with an egg, hitting myself with a snowball and (I think) enderpearling myself.

Are you 100% sure you PvP was off? If your pvp is off and you have been killed by a sword then I recommend recording it in future as, to my knowledge, this isn't something that's been done before.
Somone killed me when I was afk in my farm with pvp off
1000% i was fighting with my friend before i am sure 1000% i turned it off because he was annoying me
