My brother in Christ I was a senior mod, I never once had to approve a forum account. Every time someone asked about that, the issue was they had to go to their email and confirm it was them

I've never seen that before. It has nothing to do with us, we only have to approve name changes on the forum, that's it. So we can't really do anything here. In terms of appeals, if for whatever reason they're unable to appeal, you may appeal on their behalf and we'll take a look.
