Maybe this was a nicked staff/media rank? This name looks like it could have been generated automatically, but then again, most igns could be seen that way.

It could have been, but it is still a valid account, as they're on NameMC when you search that IGN.

There are 2 ways I can think of:
1] Way back when Minecraft was created, there was less name restrictions, therefor players could have more unique names like this.
2] Getting lucky timing /w glitches in Mojangs naming system occurring allowing people to make unique names like this.

There might be more ways that I am unsure of but I think that mostly covers it

Yeah from what I've heard about similar exploits is mostly that they are pretty rare, and they got patched fast when Mojang caught wind of it.
